What do I do next?

Presbyterian Aged Care offers staff with considerable experience delivering services and activities in both the Commonwealth Home Support Program (CHSP) and all levels of Home Care Packages (HCP). If you would like to receive Home Care services from PAC please contact our Care Support Team on 1800 722 679.

When we meet with you, we will listen to what you want to achieve from your services and activities so together we can be clear what your service goals are.  We will support you to consider aspects of care relating to your physical, emotional, psychological, social, and spiritual needs.  Once we understand your goals and needs, our staff will gain your input and direction to finalise a Care Plan, outlining the agreed services and activities that will be put in place. This will take into account all available funds, preferences for timing, and options for delivery.  Your input will ensure you’re in control and completely satisfied with the services that you’ll receive. PAC will help you to find out if other service options are more suitable to you and assist with any necessary referrals.  PAC will also explain any services that you might like to have above what your funded services will cover, and how these might be accessed by you.

To increase the likelihood of accessing Government Funded Aged Care services the recipient will need to be registered with My Aged Care. This can be done by contacting 1800 200 422 or going online at www.myagedcare.gov.au.
